Use this for over-the-counter pain relievers, ADD/ADHD medications, cough/allergy medications, etc.
Do not use for asthma medication (inhaler/nebulizer) or Epipens
Anaphylaxis Emergency Plan (Epipen/Auvi-Q)
Forms for Epipen/Auvi-Q and Benadryl for students with life-threatening allergies (Anaphylaxis) to foods, insect stings, or other allergens.
BOTH the medication form and the Epipen delegate form must be completed.
Asthma Treatment Form (Inhaler/Nebulizer)
Form for inhalers and/or nebulizer medication.
The school provides a nebulizer machine. Parents must provide the medication, tubing, and mouthpiece.
